The Doctor Who Cracked One Hand for 60 Years

Dr. Donald Unger, a California physician, spent roughly 60 years cracking the knuckles on his left hand every day while leaving his right hand alone as a control. He published his findings in 1998 in the journal Arthritis & Rheumatism. After six decades of deliberate, daily cracking on one side, neither hand showed any signs of arthritis.
Unger won an Ig Nobel Prize for the research in 2009 — the annual award given for science that "first makes people laugh, and then makes them think." His acceptance speech reportedly included a pointed suggestion that his mother reconsider her advice.
His study was small, obviously, but it was far from the only data point. Larger epidemiological studies have looked at populations of habitual knuckle crackers versus non-crackers and found no elevated rates of arthritis. The American Academy of Family Physicians has addressed the question directly. The answer is consistently the same.
So What Is That Sound?
This is where the story gets interesting, because scientists spent an embarrassingly long time disagreeing about the answer.
For decades, the dominant explanation was cavitation. The synovial fluid that lubricates your joints contains dissolved gases — primarily carbon dioxide and nitrogen. When you stretch the joint by pulling or bending the finger, the pressure inside drops rapidly. That pressure drop causes dissolved gas to come out of solution and form a bubble. The pop you hear is that bubble forming.
That explanation held for years. Then researchers started questioning it. Were the bubbles forming, or collapsing? Was it one event or two? The pop-science consensus kept shifting.
In 2015, a team at the University of Alberta used real-time MRI imaging to watch knuckle cracking as it happened. Their footage showed a bright flash inside the joint at the exact moment of the crack — which they interpreted as a gas cavity rapidly forming. The bubble, in their analysis, was the sound source, not its collapse.
Then in 2018, another research group published a mathematical model suggesting the sound is actually produced by the partial collapse of that cavity — not its formation. The debate, remarkably, is still not fully resolved.
What everyone agrees on is that the sound is gas-related and that the joint typically needs a reset period of 15 to 30 minutes before it can be cracked again — the time it takes for the gases to re-dissolve into the fluid.
Why the Arthritis Myth Took Hold
Given that the science has never supported the warning, why did it become such a durable piece of family wisdom?

There's also a surface plausibility to the concern. Joints are delicate. Arthritis is real and painful. The idea that repeatedly stressing a joint could cause long-term damage isn't crazy — it just happens to be wrong in this specific case. The intuition sounds reasonable even though the evidence doesn't support it.
And once a belief like this gets attached to parental authority and childhood memory, it becomes nearly impervious to correction. People who heard this warning as children remember it as settled fact, and they pass it on with the same confidence it was given to them.
What Actually Causes Arthritis
Since the knuckle-cracking myth takes up so much conversational real estate on this topic, it's worth being clear about what actually does contribute to arthritis — because those factors are both real and significantly underappreciated.
Osteoarthritis, the most common form, is driven primarily by age-related cartilage breakdown, genetics, prior joint injuries, repetitive stress from certain types of work or athletics, and carrying excess body weight — which increases the mechanical load on joints like knees and hips. Rheumatoid arthritis is an autoimmune condition with a completely different mechanism.
None of these causes involve the casual stretching of a finger joint. The cartilage is not being damaged by the pop. The joint space is not being narrowed. The inflammatory cascade that characterizes arthritis is not being triggered.
There is one minor caveat worth mentioning: some research has suggested that very long-term, very frequent knuckle cracking might be associated with some soft tissue laxity — essentially, the ligaments around the joint may loosen slightly over time. This doesn't cause arthritis, and it doesn't cause pain, but it's a small signal that the habit isn't entirely without any effect. It just isn't the effect everyone warned you about.
